Register a Home Birth

Registering a home birth in Henderson County must be completed within one year of the child’s birth.

The following documents are required for appointment:

1. Fill out the Home Birth Registration Packet completely (no blanks) and return by mail, fax, or email to:

2. Obtain a statement from someone who can verify the mother was pregnant. This can be a coworker, pastor, neighbor, friend, or anyone who is not a relative. This document must be notarized (you may use the Home Birth Pregnancy Affidavit). You can also use a copy of an ultrasound if it has a date and the mother’s name printed on it.

3. Obtain a recent document that verifies your residence (utility bill, tax record, etc.).

4. Valid picture ID for both the mother and father (if the father’s name will be added to the certificate)

You will receive a call from our office to make an appointment to come in and complete the registration of your home birth. The mother may not be the only person required to be present. The father may also be required if his name is to be added to the certificate.
