Description

The Henderson County Department of Public Health is a public agency that provides health services to advance its stated mission to “promote, protect, and advance the health and wellness of our community”. The primary purpose of the care management unit is to decrease barriers to health care and improve health outcomes in pregnant women and children.

As a Case Manager for At-Risk Children (CMARC), this employee provides care management services to Medicaid-eligible and uninsured children in Henderson County from birth to age five years. The Care Manager in this position works with children and families who have been identified as having special health care needs, exposed to toxic stress or are in foster care placement to decrease social and health care barriers and improve health outcomes.

Schedule

The normal work schedule will be 8:00 a.m. – 4:30 p.m., Monday through Friday. Evening and weekend work may be required to meet patient needs and to assist with public health events as requested.

Qualifications

Considerable knowledge of and skill in the application of nursing theory, practices, principles and techniques employed in the field of public health and related programs; considerable knowledge of and the ability to apply principles and practices of public health; knowledge of available resources and organizations and the ability to coordinate these as needed; general knowledge of current social and economic problems relating to public health, including health disparities; ability to plan, coordinate and oversee work of others; ability to deal tactfully with others and to exercise good judgement in appraising situations and making decisions; ability to work in partnership with patients and with other service providers to elicit needed information and to maintain effective working relationships; ability to record accurately services rendered and to interpret and explain records, reports and medical instructions; adequate computer skills to allow communication, patient record documentation, and accessing of information. 

Preferred Qualifications

Previous care management experience. Previous experience in pediatric or obstetrical nursing. Spanish language skills.   

Required Minimum Training and Experience

A current N.C. Registered Nurse license, graduation from an accredited school of professional nursing and one year of professional nursing experience.

Minimum Annual Salary: $70,019.87

Grade:          79A

Hiring salary will be determined based on qualifications and internal equity.
