Subdivision Forms

  1. Subdivision Application Form - Must be submitted with all special, minor and major subdivision plans

  2. Appointment of Agent Form - May be submitted with a subdivision application to designate an agent who will represent the property owner in all matters
  3. Minor Subdivision Form - Must be submitted with all minor subdivision plans

  4. Master Plan Requirements Form - Must be submitted with all major subdivision master plans

  5. Development Plan Requirements Form - Must be submitted with all major subdivision development plans
  6. Emergency Services Impact Report Form - Must be submitted with all major subdivision master plans where there are 100 or more lots or the subdivision is proposing more than 50 units located more than five (5) road miles from a fire station

  7. Traffic Impact Study

    (No specific form is required, however the report must meet the requirements of Chapter 42A). Must be sumitted with all major subdivision master plans where there are 100 or more lots or units and/or the proposed development will generate an average daily traffic count of 1,000 plus vehicles per day or 100 plus trips during peak hour traffic

  8. Street Tree Plan -

    (No specific form is required, however a plan is to be provided that meets the requirements of Chapter 42A). Must be submitted with all major subdivision master plans.

  9. Final Plat Requirements Form - Must be submitted with all final plats for recordation

  10. Affidavit of Understanding of Farmland Preservation District Form - Must be submitted (when necessary) with major subdivision plans

  11. Application for Improvement Guarantees Forms - Must be submitted with improvement guarantee materials
  12. Subdivision Certificate of Understanding Form - Must be submitted for any subdivision utilizing driveway easements in lieu of rights-of-way and associated roads.
