Duties and Responsibilities: The caseload for this position will primarily consist of clients receiving In Home Aide and Special Assistance In Home services. This position assesses the client’s needs, arranges for services including in-home assistance, health support, individual and family adjustment services, representative payee services, and Special Assistance In Home. This position provides case management to elderly and/or disabled adults and will participate in Adult Services intake coverage duties and in the rotation of on-call duties with other social workers in the Adult Services units. This position requires time spent in the office as well as visiting clients outside of office settings.
Qualifications: Knowledge of interpersonal communications and several methods of interpersonal help or treatment. Knowledge of racial, ethnic, and other cultural groups in society including their values and lifestyles is necessary. Significant skills in counseling, interviewing, and listening to others with understanding and purpose; in creating and maintaining professional helping relationships; in developing resources where none are readily available. Knowledge, skill and ability to provide case management services to clients to ensure needs are met; to provide coordination of services to avoid duplication of services, maximization of service delivery process, and delay pre-mature placement outside the home.
Bachelors in Social Work from an accredited school of social work, or a four-year college or university degree in a human services field plus one year of social work or counseling experience, or a four-year college or university degree plus two years of social work or counseling experience.
Must successfully complete any/all State mandated training relevant to this position. At least one year’s experience working with the target population is strongly preferred. Experience with Medicaid program is helpful. Some training or experience in the medical field, such as nursing, is helpful. Knowledge of community resources is strongly preferred.
