Reporting A Disease

To Report a Disease:

Download the NC Communicable Disease Report Form or call (828) 694-6019. Physicians or their designee are to complete the one page form and then FAX to 828-697-4504.

Investigations
We follow protocols established by the Centers for Disease Control and the North Carolina Division of Public Health. Protocols for disease investigation and statistical data for North Carolina and Henderson County are available on the Division's website.

Reporting
With the rollout of the North Carolina Electronic Disease Surveillance System (NCEDSS), the NC Communicable Disease Manual has been revised with reporting forms, compatible with NCEDSS, which replace previous reporting cards and surveillance forms.

 

The form may also be sent by mail to the Henderson County Department of Public Health, Communicable Disease Control Section. The mailing address is printed on the Disease Report form.

Here are all currently reportable diseases with the required reporting time, 24 hours or 7 days. The complete revised North Carolina Communicable Disease Manual is available on the General Communicable Disease Control website.

What diseases are reportable?

Diseases Reportable within 24 Hours ​​

Telephone reports are requested within 24 hours for diseases of unusual significance, incidence, or occurrence which may merit an epidemiological evaluation including foodborne and waterborne outbreaks where a common source is suspected.

For diseases and conditions required to be reported within 24 hours, the initial report shall be made by telephone to the appropriate person listed on the  Communicable Disease Reporting Roster. 

A  ​NC Communicable Disease Reporti​ng Form should also be completed and faxed.

 

Telephone reports should include:​

date of onset
name of person making report
telephone number of person making the report
laboratory confirmation, if available​

patient name
patient address
​patient age
patient sex
patient race

Diseases Reportable Within 7 Days

For diseases and conditions that must be reported within 7 days, a ​Communicable Disease Reporti​ng Form ​​should be completed and mailed to the appropriate person and Health Department site.​​ ​​
